POSITION PURPOSE/SUMMARY: As a successful Hospice of San Joaquin Hospice Liaison, you will build and maintain collaborative relationships with assisted living facilities, hospitals, physician offices and other community referral sources. It will be your responsibility to grow the agency’s reputation in the community through strategic outreach to community partners. We want you to become part of our team culture and collaborate fully with our top-notch clinical staff on a daily basis.
