Referrals will be submitted directly to the Independent Contractor by community partners or identified through outreach activities and will be forwarded to the Agency Intake Coordinator within 24 hours of receipt. • Submit a monthly, signed activity summary and invoice to the Agency Executive Director documenting outreach activities, referral sources contacted, referrals received, intakes scheduled, and completed intakes for that month. • May participate in Agency meetings or planning initiatives related to referral development and community outreach, as requested by the Executive Director. • Strong communication and relationship-building skills with the ability to engage community partners and referral sources. • Ability to coordinate scheduling and follow up with families and referral sources to support intake completion.
